Due to the confidentiality of our records, all records are protected by both Federal and State Privacy laws. If you wish to check your records regarding financial responsibility requirements, you must appear in person with proper identification. Confidential information is not given over the telephone. Due to the extensive research necessary to retrieve your file and answer questions you may have when you appear in person, it is important that you arrive no later than 3:30pm. This will allow us sufficient time to recall your records, explain the reason for the financial responsibility requirement and answer any specific questions you may have.
If you have received an order of security requirement or suspension notice, the order was issued as a result of your involvement in an accident which the police officer has classified as a major accident. You may purchase a copy of this police accident report from the records division of the Maui Police Department. If at the time of the accident you possessed a valid insurance policy, refer to item 1 on the reverse side of the order you received. Contact the insurance company who issued your policy at the time of the accident and they will assist you with the submission of the SR-21 Form. Only insurance companies have the SR-21 Form. This office does not provide this form.
If you have received an order of requirement for proof of financial responsibility or suspension, the order was issued as a result of a traffic court conviction. If you are required or choose to submit an SR-22 Form, please contact your insurance company. They will be able to assist you with the submission of this form. This office does not provide this form.
