Pursuant to Chapter 103-D, HRS, proposals are being accepted for supplying, furnishing and paying for all labor, materials, tools, transportation and equipment necessary to construct in place complete, all work shown and called for in the specifications for the "REPLACEMENT OF THE EXISTING INDIVIDUAL WASTEWATER SYSTEM AT THE HOOLEHUA FIRE STATION", IFB NO. 08-09/P-25 Department of Fire and Public Safety, County of Maui.
Sealed proposals shall be received and time stamped by the Bid Receiver, Purchasing Division, Department of Finance, Wells Street Professional Center, 2145 Wells St., Suite 104, Wailuku, Maui, Hawaii 96793 by 2:00 p.m. on FRIDAY, OCTOBER 31, 2008, and shall be publicly opened thereafter.
Specifications and Proposal Form may be obtained at the Purchasing Division, Department of Finance, at the address above.
All prospective bidders shall possess the appropriate Contractor’s License, with appropriate subcontractor(s), held in the State of Hawaii.
The Contractor and his Subcontractor(s), if any, shall comply with all applicable revisions of the prevailing wage schedule as required under Chapter 104, H.R.S.
A mandatory pre-bid site inspection is required for all prospective bidders. Bidders shall contact Assistant Fire Chief Alan Pascua at (808) 270-6211, Deputy Chief Robert Shimada at (808) 270-7561 or Fire Chief Jeff Murray at (808) 270-7561 to schedule the inspection. The inspection shall be completed at least seven (7) business days prior to the bid opening date. Bidders shall sign-in at the time of the inspection. Bidders shall notify the Captain on duty at Hoolehua Fire Station (808)567-6555 upon arrival at the site to be signed-in.
No proposal will be considered unless accompanied by one of the following forms of bidders’ security: cash, a cashier’s check or certified check on a bank insured by the Federal Deposit Insurance Corporation, or a bidder’s bond executed by an approved surety insurer, made payable to the Director of Finance in the manner, form, and amount required by Section 103D-323, Hawaii Revised Statues. The proposal guaranty shall be equal to at least five percent (5%) of the total bid amount submitted.
